Who Can Apply For UDID Card

This card is available to everyone with a disability as defined by the Rights of Persons with Disabilities Act. This covers a diverse spectrum of physical, sensory, intellectual and mental disabilities.

There is no specific age criterion for applying and new applicants as well as people with an old disability certificate are eligible to apply for this card. Transgender people also have their own scheme for applying for a certificate and identity card on the same portal.

Documents Required For UDID Card

Prior to beginning the application, have these documents on hand.
If you have a disability certificate from before, please bring it with you.
Show proof of identity, e.g. voter ID, Aadhaar card or PAN card
Provide proof address, this can also be your Aadhaar card in most of the cases.
A recent, passport-size photograph
Mobile Number for verification and updates + email ID
If you don’t already have a disability certificate, it’s okay as it will be issued in the near future. You may apply and a certificate of medical assessment will be issued following the same process.

How To Apply For UDID Card Step By Step

All applications are online and can be completed from home. Simply follow the steps below with care.

Step 1: To visit the official website of the Swavlamban Card at swavlambancard.gov.in. Always look for the .gov.in URL; this will protect you from getting scammed by fake websites.

Step 2: Go to the home page and click on “Apply for Disability Certificate and UDID Card”.

Step 3: Decide whether you are applying to an existing enrollment number or applying fresh and go ahead with the process.

Step 4: Enter the personal information like full name, Date of Birth, gender, mobile number, email ID, Aadhaar number etc.

Step 5: Provide information on the nature of your disability and its severity, your home address, and other information needed.

Step 6: Submit the scanned documents of your disability certificate (where applicable) and identity proof, address proof and a passport-size photo.

Step 7: Carefully review all the details and click on submit if all things look correct.

Once submitted, if you do not already have a disability certificate, your application is then forwarded to the relevant Chief Medical Officer or Medical Board. Then, it will be necessary to go to the specified hospital for the evaluation. After an assessment, your disability certificate and UDID card are issued at the same time.

Once again, go back to the same portal swavlambancard.gov.in and click on Track Application Status.

Write down your enrollment/ application number and date of birth.

Enter the captcha code on the screen and click Submit.
You will then see how far you are in the application, whether you are still in the process of being reviewed, being sent for medical assessment or you have already been accepted and your application is ready to download.

Now you can also log in using your credentials and then choose your UDID Card Download option from your account.

This card will come in PDF format that you can save on your computer or phone and also print whenever needed for official use.

And you have to keep your registered mobile number active at all times, since most updates and OTP related communication depend on that number.
